If you call outside of our receptionist's hours, please note the providers do not answer the phone while in session. However, we monitor voicemail closely and work hard to return all calls within 24 to 48 business hours. To make the most of your first visit with us, please remember to:

1. Bring your insurance benefits card and a current government-issued photo ID. If military or a military dependent, all you need is a copy of your military ID. If retired military/dependent, please bring your military ID and insurance benefits card for other insurance coverage you may have, if applicable

2. Verify your copay and any unmet deductibles you may have. If we cannot verify this information during your visit you will be asked to pay the entire amount due at the time of your visit and any amount overpaid will be refunded to you when we receive payment from your insurer

3. To maximize the time spent with your provider during your first visit, please arrive 30 minutes prior to your appointment time to complete your initial paperwork or print and fill out this form then bring it with you
